It is the policy of our office to provide excellent health care in a friendly, professional atmosphere. We offer evening office hours for our patients' convenience. Patients can be assured of immediate attention for emergencies as we can be reached 24 hours a day, seven days a week.
Patient Information
Please note that you will receive a text and email three days prior to your appointment from Phreesia, prompting you to complete your new patient registration packet. If for any reason you can't complete forms electronically, please complete and print the forms provided below to bring to your appointment.
If you cannot do either, please arrive 15 minutes prior to your appointment to complete the paperwork.
Your options are:

- Print and complete the forms listed below
- Using Adobe Acrobat Reader, download, fill in and print the forms below
- Request the forms to be mailed to your home
Office Hours
Our office hours vary depending on the provider and the location, but at least one provider is always available.
Our regular office hours:
Monday through Thursday: 8:30am - 6:00pm
Friday: 9:00am - 2:00pm

If you are uninsured, you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
